Cumberland View’s in-home care Melbourne can help you with these services. In-home care is coordinated care and services to help you live independently in your own home for as long as possible.

Home care packages are designed for seniors and a young person with a disability so they can remain safe at home.

The care is provided by accredited healthcare professional ‘Cumberland View Home Care’ that coordinates the in-home care Melbourne, to come to your home and help you with certain activities that you may find difficult. Assessment

To qualify for a Government funded home care package, you must undergo an aged care assessment. During your assessment, your assessor will discuss your current needs to identify which home care package service level best suits you. There are four levels of home care packages with different care needs – Level 1 being basic to Level 4 being high.

To find out if you are eligible, call My Aged Care on 1800 200 422. If your care needs appear to indicate that you might need in home care services, you will be referred to an Aged Care Assessment Team (ACAT) to complete an assessment. Your local ACAT member is usually a nurse, social worker or other health care professional. These assessments are free.

If you are eligible, you will receive an approval letter. You will then be placed on the national queue until a suitable home care package becomes available.

Cumberland View’s Home Care Manager will work closely with you to develop a care plan and determine what care and services best meet your needs. They will manage your in-home care Melbourne services. They will also take care of your budget, funding and invoicing so you can focus of enjoying life.

The Australian Government subsidies the home care packages but you may need to contribution towards the cost of your care. This amount depends on the home care package level that you require and your personal circumstances.

There are two types of fees you may be required to pay. These are:

• A basic daily fee

• An income-tested care fee (some people may be asked to pay this, but it will depend on their individual circumstances)

The basic daily fee for a home care package is 17.5% of the single person rate of the basic age pension. This may change after the 19th March 2019.

If you are paid a pension that is means tested, you will not need to lodge an income assessment form as the Government already has enough information to work this out. These pensions include: Age Pension, Disability support Pension, Service Pension and Income support Supplement.

If you are paid a pension that is not means tested or if you are a self-funded retiree, then you will need to lodge an assessment form with the Government.

You will be advised by the Government the amount you may be asked to contribute towards your home care package costs. Fees will be discussed between you and Cumberland View’s Home Care Manager.

Assigned a Home Care Package

When a home care package becomes available based on your position in the national queue, you will receive a letter from the Government to let you know you have been assigned a home care package. You then have 56 days to enter into a Home Care Agreement with Cumberland View Home Care.

Entering into a Home Care Agreement

Having chosen Cumberland View Home Care as your provider, you will be required to enter into a Home Care Agreement. Details of the agreement can be discussed with the Home Care Manager. You will then work together to develop a Care Plan that best suits your needs and personal budget. It is advisable to seek legal advice (if you wish) before signing the agreement.
